If you are planning to move to another state we would like to help make the transition as smooth as possible. We recommend you let your provider know first. Then reach out to customer support team to inform us which state you are moving to. 

We will check your state and let you know if there are available appointments with a provider. If so, we will book an initial appointment, which will be discounted to a follow up ($59). Please note you will need to fill out a new intake form and the new provider cannot prescribe medication prior to having that first online visit. 

If there are no available providers, we will send a release request for your medical records. You will need to complete the form and let us know when it is finished. Then, we'll release your records to the secure client portal after this has been received or it can be faxed to your new provider.
