Prior to your initial appointment visit we will provide you with an assessment and intake forms. This is generally enough information for your provider to diagnose and provide an individualized treatment care plan for you. Having your medical history and notes will be handy while filling out the intake forms. It will also be useful to forward this information or any pertinent medical records to your provider prior to your visit. These documents can be uploaded to your secure client portal after you have booked your initial visit and registered for the portal.
