Prior to your visit, you must show proper identification. We only accept a valid photo ID that is not expired. You will not receive a prescription without proper identification. Keep in mind that most pharmacies will not dispense controlled medication without an in-state photo ID.
Some providers may accept a current lease or other proof of residency. Should you have any concerns about your photo ID or proof of residency for your state please message directly with the provider prior to your visit. This can be done in the client portal after you have completed registration.
***Please note: If you are not able to provide proper identification, it is at the provider's sole discretion to cancel your upcoming appointment.***
