If you have been diagnosed by a mental health provider in the past 5 years and have been on medication, you will still need to begin with initial appointment visit. During your visit your provider will carefully review your medical history and evaluate your current symptoms, as well as help you develop an optimized treatment plan.
If you had your diagnosis a while ago, or have not been receiving any treatment, you will also start with the initial appointment visit. The initial visit is required regardless of your medical history.
You will all receive an intake form before your visit. Be sure to outline your relevant medical history including any medications you have previously been prescribed for your provider to review. Your provider will thoroughly review your symptoms and mental health history before your appointment.
